WE ARE NOW SEEING NEW PATIENTS ONLINE THROUGH TELEHEALTH FOR YOUR CONVENIENCE AND TO PROTECT ALL PATIENTS FROM POTENTIALLY BEING EXPOSED TO THE CORONAVIRUS
We are excited to help you with your health goals. A couple things to remind you of:
1.Upon scheduling your appointment, you will receive an email with a link to set up your patient portal. Please complete the information and health history in your portal as well as our new patient form, at least 2 days before your appointment.
Below, you will find a link to our new patient forms: Weight Loss Form and Diabetes Form.
Complete the form that applies to the reason for your visit and send it back to us through your patient portal.
2. To be safe, expect your first visit to last about 1 hour. Just like at any doctor’s office, there is going to be a little bit of paperwork to do. So, please arrive 20 minutes before your appointment.
3.Please bring your medications, a photo ID and your insurance card(s) with you.
4. Please send us a copy of any recent labs through your patient portal or ask your provider to fax it to us at 252-364-8874 prior to your appointment. The following Blood test results are needed before your second appointment: Comprehensive Metabolic Profile, B12 and Vitamin D, 25-OH, Hemoglobin A1C, CBC w/Diff, Thyroid Profile (TSH, free T4), fasting Lipid Profile Insulin with glucose Level. Dr Nachida Hamidi-Sitouah will order the missing labs at your first appointment. Please be aware that some lab results are needed before prescribing medications and sending us your recent labs will prevent any delay in this process.
5. Please note: we have a 48 hour cancellation policy. If you need to cancel for any reason, please let us know at least 48 hours before your appointment or there will be a $70 cancellation fee. If you need to reach us prior to your first appointment, you can email us back, or call us at 252-227-0080.
This policy applies to all kinds of appointments including: in-person and online initial medical visits and follow ups, as well as coaching visits.
All missed appointment fees must be paid prior to your next visit. Exceptions may apply!
After 3 no show/late cancellations, you will be discharged from the program for 1 year until you can make it a priority.
We understand that unexpected things happen, but accountability is number 1 key for success in reaching your health goals.
